By filling in the form below, you will agree to get performed the Polymerase Chain Reaction test for Atadenovirus and notify PATS prior to sending off the fecal samples to the Research Associates Laboratory in Dallas, Texas. You will agree to complete honesty and integrity in order to maintain the highest level of standards possible. Each member will be held accountable for providing the proper documentation as requested by PATS. I grant PATS the permission to view and to use the results for statistics, anonymously, on our website or for medical reports as needed. I do understand that these results will be used for the sole intent and purpose of data compilation for science and research studies as well as for medical information and purposes towards the future. We realize that your privacy and discretion are very important to you and we will make every effort to ensure that they are not violated in any way.

Membership into PATS is completely voluntary and we will not be held responsible for any monetary losses, business or personal. This membership is for Atadenovirus testing only, and does not reflect the breeders' husbandry. Any husbandry and or care concerns need to be addressed with the breeder prior to any purchases. We are not to be held liable for any errors that may or may not occur with the Atadenovirus testing. It is the responsibility of the breeder to ensure that any of the samples are sent properly and free from contamination.
